We are seeking a dedicated and highly skilled Advanced Clinical Practitioner (ACP) to join our collaborative and patient-centered team. As an ACP, you will provide advanced care, leadership, and innovation within our clinical practice while ensuring a compassionate and inclusive approach to patient care. Responsibilities
- Provide advanced, evidence-based clinical care to patients in a multidisciplinary setting while supporting the principles of equality, diversity, and inclusion.
- Perform comprehensive assessments, diagnosis, and management of patients with complex healthcare needs.
- Collaborate with physicians, nurses, allied health professionals, and external partners to ensure the delivery of holistic, patient-focused care.
- Lead and participate in clinical decision-making processes and case discussions.
- Develop, implement, and evaluate individualized treatment plans in collaboration with the healthcare team.
- Support the education and mentorship of junior staff, students, and peers, fostering a culture of continuous learning and professional development.
- Lead quality improvement initiatives to enhance clinical services and patient care outcomes.
- Maintain accurate and detailed clinical documentation in line with organizational policies and compliance standards.
- Stay current with advancements in healthcare, evidence-based practices, and professional regulations relevant to your role.
Qualifications
- Registered Nurse with a valid Nursing and Midwifery Council (NMC) registration or equivalent.
- A Master’s degree in Advanced Clinical Practice or working towards achieving this qualification.
- Recognized Advanced Clinical Practitioner credentials or certifications.
- Extensive clinical experience in advanced nursing or a related area.
- Strong understanding of advanced clinical assessment, diagnostics, and decision-making principles.
- Excellent communication and teamwork skills, with a commitment to fostering a respectful, supportive, and inclusive environment for patients and staff.
- Ability to work autonomously and confidently under pressure while maintaining a high level of clinical competence.
- A passion for patient-centered care, innovation, and continuous quality improvement.
Day-to-day
- Conduct advanced clinical assessments, diagnose conditions, and develop management plans for a wide range of patients, utilizing your advanced professional skills and judgment.
- Provide mentorship and coaching to junior staff, ensuring equitable access to learning opportunities and career progression.
- Collaborate with patients and their families to create care plans that reflect their individual needs, preferences, and cultural values.
- Work alongside healthcare colleagues to identify and implement strategies for improving care pathways and patient satisfaction.
- Attend multidisciplinary meetings to share expertise and contribute to clinical decision-making processes.
- Keep up-to-date with the latest guidelines and best practices, and incorporate these into your daily practice.
